Whose left?
- A "mass on the left" means the patient's left, always. Anatomy has a precise language because vague words cause real harm.
- Learn the grammar once, and every later description, scan and operation note becomes clear.
Directions
- Every direction assumes the anatomical position 解剖学姿势: standing, facing you, palms forward. Superior/inferior is up/down; anterior/posterior is front/back.
- Medial/lateral is towards/away from the midline. Proximal 近侧 and distal 远侧 are nearer to and further from where a limb attaches.
Match each pair of terms to what it describes.

Each pair is relative to the anatomical position, so it means the same thing whatever the patient's posture.
Three planes
- The sagittal plane 矢状面 runs front to back and splits left from right. The coronal plane 冠状面 runs side to side and splits front from back.
- The transverse plane 横断面 is a horizontal cross-section that splits upper from lower. Every scan you will read is cut in one of these.

Which plane splits the body into left and right parts?

The sagittal plane runs front to back, so it separates left from right. Coronal separates front from back; transverse, top from bottom.
How a CT slice is shown
- A CT or MRI report names the plane ("axial T2", "coronal CT") and the side. An axial slice is shown as if you stand at the patient's feet looking up.
- So the patient's right appears on the left of the image. Applied every time, this one rule lets two readers agree about a lesion without seeing the patient.
On a standard axial CT slice, where does the patient's right side appear?

Axial slices are viewed as if from the patient's feet, so the patient's right is on the image's left. The convention is the same everywhere.
Worked example: the direction drill
- The elbow is distal to the shoulder and proximal to the wrist. The sternum is anterior to the heart, which is anterior to the oesophagus.
- Drill these until they are automatic: each pair replaces a whole sentence of description.
Put these chest structures in order from anterior to posterior.

The heart sits right behind the sternum, the oesophagus runs behind the heart, and the spine is at the back.
Worked example: naming the site
- (Fictional vignette) A tension pneumothorax is relieved with a needle at "left, second intercostal space, mid-clavicular line".
- The grammar is the safety system: side first, a landmark in two directions, and depth counted by rib spaces, not centimetres.
In a clinical note, "left second intercostal space" means the left side as the doctor faces the patient.

Sides always mean the patient's own left and right, whoever is looking.
Three common slips
- Observer's side for patient's side. On an axial image, the patient's right is on your left.
- Mixing reference points. Medial/lateral are measured from the midline; proximal/distal from where the limb attaches.
- "Coronal means from the top." The coronal plane is vertical, parallel to the coronal suture, and splits front from back.
A coronal MRI slice of the head passes through both ears. Which parts of the head does it separate?
难度 1/5 · 今日首次完成可获 100 金币及少量额外奖励
The coronal plane is vertical and runs ear to ear, splitting the body into front and back parts.
Recap
- All directions assume the anatomical position.
- Sagittal splits left/right, coronal splits front/back, transverse splits top/bottom.
- On axial images, the patient's right is on the viewer's left.
